OUT OF TOWN PATIENTS AND LOCAL PATIENTS
We know there is a lot of planning when traveling to our office by plane or car, so we want to make it as smooth as possible for you. Please read the information below so you know what to expect when traveling to our office and then contact us by email at info@oasisdentalmilton.com or phone at 905-876-2747.
STEP 1: Virtual Video Consultation (Video Call)
We will provide a questionnaire form to collect information about your condition and symptoms. Your questionnaire needs to be completed and submitted back prior to the video consultation and please provide as much detail as possible in order for Dr. Soordhar to fully understand your concerns and problems.
The initial in-office consultation, Dr. Soordhar will perform a clinical exam to determine the dental relation (underbite, overbite, crossbite, narrow jaw, teeth crowding, deep bite), jaw function, muscle dysfunction, TMJ disc dysfunction, and pain sources. After this is completed Dr. Soordhar will determine what imaging is required (Panorex, Cephalomtric, TMJ MRI and/or TMJ CBCT) and you will be required to get this completed if needed.
STEP 2: Initial Consultation Appointment (In-Office)
STEP 3: Treatment Plan Consultation (Video call)
Based on your Virtual Consultation interview (Step 1), Initial Consultation and Imaging (Panorex/Cephalometric/MRI/CBCT) (Step 2), Dr. Soordhar will review your treatment options for your treatment.
STEP 4: Record Workup Appointment (In-Office)
During this appointment, full records will be taken that consist of dental models, posture analysis, clinical photos, and determining the optimal jaw position for your treatment. Your appliances will be prepared by the lab to be delivered in 3-4 weeks.
STEP 5: Insertion Delivery of Appliance(s) (In-Office)
Dr. Soordhar will insert your appliance, adjust it as needed and record measurements to track your treatment progress. Dr. Soordhar will do his best to make sure you are as comfortable as possible before you go home.
STEP 6: Future Appointments as Needed (In-Office)
Follow up appointments every 4 to 8 weeks will be required to adjust your appliance(s) in order for your treatment to produce results. The importance of these follow up appointments is critical to allow adjustments to your appliance from it wearing down that could result in an increase in your symptoms.
